Membership is not insurance. The annual dental fee is collected at enrollment and renews yearly. Benefits are nontransferable. Savings apply only to eligible services and cannot be combined with another package or promotion.

The first monthly Passport payment is collected at enrollment. Cancel the month-to-month Passport before your next billing date; your annual dental membership remains active for its yearly term.

Up to two nurse practitioner lab-result evaluations are included per membership year. Lab draws, testing, processing and Function Health fees remain separate.
Important details
- Skincare benefits may accumulate through payment 12 but must be used while the Passport is active.
- Peptides are provider-selected based on clinical appropriateness and availability.
- Savings exclude compounded prescriptions, additional peptides, retail products, laboratory fees, membership payments and discounted packages or promotions.
- Benefits are nontransferable; unused benefits are forfeited when the Passport ends.
